Long Term Care Facilities Support
We offer specific services ideal for long term care facilities. From specialized long-term care staff to wholesale access, we ensure your staff and the people you serve have everything they need.
Custom Pill Packaging
We offer more than just your standard pill bottles! Blister packs and strip packaging are available that make is easier for individuals to access or remember to take their medication. This is also ideal for caregivers who manage other's medications.
Medication Synchronization
We work with you to ensure your medications are all ready at the same time, ensuring an easy pickup or delivery process. For long-term care facilities we can coordinate all your patients medications for one convenient delivery.
Delivery and Mail Order
We offer free area delivery on most prescriptions, for both individuals and for long term care facilities.
Immunizations
We offer walk in immunizations at our location and vaccine clinics that travel to you.

Leadership Team
Prime Care Coordination is led by Kate Wagner. Kate previously served as Vice President of CDS Life Transitions, providing day to day direction and support to CDS Monarch, Unistel, and Warrior Salute Veteran Services.
Kate began her career at CDS Monarch in 2012 as a registered nurse for the organization, progressing quickly to assume senior management responsibilities. In her role as Vice President of Operations at CDS Life Transitions, Kate has been responsible for more than 400 employees and over a $35 million budget. Ms. Wagner has overseen the care and support of more than 1500 people with IDD, gaining expertise in administration and leadership, nursing and clinical services, facilities, quality management and driving systemic improvements. Kate is a Registered Nurse and recently received her Masters of Science and Health Administration. She has a keen focus on the delivery of services to the individuals and families Prime Care Coordination serves and appreciates the role of PCC in their lives.
Terria Thompson has worked in the field since 2003, helping to serve people with intellectual and developmental disabilities and ensure they receive the highest level of care. Terria began her career as a Direct Support Professional, but has acted as a Director of Person-Centered Services at CDS Monarch since 2009, giving her over 16 years of supervisory experience.
Terria has dedicated her career to caring for the individuals and has demonstrated strong leadership skills in building and developing strong programs. Terria has a Master’s degree in Public Administration and among many skills she brings to this role, has extensive experience with quality improvement, fiscal compliance, performance management, strategic planning, project management, and training and development.
Terria will be overseeing Care Coordination Operations as well as Benefits and Entitlements.
Daniel has worked at Prime Care Coordination for over two years. He has a Bachelors Degree in Human Services from Empire State University and an Associates Degree in Business from Jamestown Community College.

Our History
The process began as we, a group of local care providers, worked together to obtain an Article 44 license. With this license, iCircle was created to assist anyone needing Medicaid Managed Long-Term Care (MLTC) services. In 2017, after nearly two years of providing managed care services, Prime Care Coordination was established, applying the same approach to help people with intellectual and developmental disabilities obtain quality care coordination services.
To be the best care coordination organization, we are committed to the same things that matter to our partner agencies and the individuals and families we serve. Diversity is a priority. We work with a wide range of partner agencies to serve a diverse population, and we want to make everyone feel comfortable with the services they receive. Our team is also made up of staff who help those we serve feel most at home.
The Prime Care Regional Offices and Care Managers are local and actively involved in their communities, continuously developing relationships with the agencies you work with on a daily basis. We are committed to a family-like environment in which we consistently and continuously work to deliver the best care possible for each individual.
